A public records request was submitted for the Palisades After-Action Review, including all associated draft documents. In response, the Department has released the requested records, including draft versions of the report.
The materials are available at the link below.
Please be advised -- drafts of the after-action report are working documents, not final findings. They often include preliminary analysis and internal discussion that are refined through review. Releasing drafts without context can create confusion or misinterpretation, particularly when those materials were never intended to represent final conclusions.
Upon assuming the role of Fire Chief, Chief Moore immediately directed an independent review of the incident to ensure a thorough, transparent examination of what occurred. The focus is on learning from the event, improving systems, and ensuring the department is better prepared moving forward.
The After-Action Review is a tool the Fire Department uses to look back at major incidents, understand what worked, what didn't, and how we can improve. The focus on this independent review is on learning and strengthening our response, not assigning blame. The goal is to make sure we continue to improve how we protect the public and support our firefighters.
Please be advised any responses to questions will be considered following the conclusion of the related legal proceedings and in accordance with legal guidance at that time. We appreciate your understanding.https://lacity.nextrequest.com/requests/25-16549
