Johnson & Johnson (JNJ) stock trades at 29.0 times its last twelve months of earnings (normalized profit plus stock-based pay after tax). Meanwhile, its price against sales ended September at the highest level of any quarter-end in ten years. That is a lot to pay for profit already booked, but the shares also buy the next two years of profit. So what does that price look like on the profit expected for fiscal 2026 and fiscal 2027?

What Johnson & Johnson Costs On The Profit Ahead

Based on the consensus forecast, Johnson & Johnson carries a market value of 23.6 times its expected profit for fiscal 2026. On the forecast for fiscal 2027, that multiple is 22.7 times profit. Those prices reflect the profit the forecast expects, provided that profit arrives. The fiscal 2027 forecast is the less certain of the two, given that the gap between its highest and lowest earnings per share estimates amounts to 14.0% of the consensus estimate.

The Growth Johnson & Johnson Has To Deliver

For fiscal 2026, the consensus forecast aligns closely with what management itself expects. The $101 billion revenue estimate matches the company guide from the October 13, 2026 release. Its expected $11.11 of earnings per share also falls within management's adjusted guide of $11.05 to $11.15. Management lowered the midpoint of that guide to $11.10, down from the $11.68 projected in its July release.

Looking ahead to fiscal 2027, the consensus forecast points to revenue growing 6.6%, compared to the 7.8% Johnson & Johnson delivered over the last twelve months.

The consensus forecast does not assume wider margins, either. Its net margin for fiscal 2027 is 26.2%, trailing the 28.5% Johnson & Johnson reported for fiscal 2025. The forecast may count adjusted profit, while that 28.5% is reported profit, so the two margins may not be measured the same way. Forecasted profit grows 3.9% in fiscal 2027, a slower pace than sales.

Where Johnson & Johnson's Forecast Could Fall Short

One possible shortfall involves a cost management has already assigned to fiscal 2027 and asked analysts to build into their models. On the fiscal Q3 2026 call, management said the Firefly Bio and Cell BioMedicines deals would cut fiscal 2027 adjusted earnings by $1.36 a share, compared to $0.64 in fiscal 2026. Management also said its base margin continues to improve without those deals. Any consensus forecast that has not incorporated the full fiscal 2027 cost would come down.

The second potential shortfall is MedTech, the segment that made up 35.9% of revenue in fiscal 2025. Operational sales in the division grew 2.5% in fiscal Q3 2026, and management said that performance fell short of its own expectations for the segment. Company sales growth slowed from 9.9% in fiscal Q1 2026 to 5.9% in fiscal Q3 2026, a pace already below what the consensus forecast has for fiscal 2027. If MedTech stays slow, the fiscal 2027 sales forecast would come down with it.

If the profit in the consensus forecast arrives, the forward multiples offer a fair picture of what you pay for Johnson & Johnson today. If it falls short, most likely through the deal cost or a slow MedTech business, the stock would be a riskier bet, priced for profit the company has not earned. MedTech sales growth, in the fourth-quarter report Johnson & Johnson is expected to release in January 2027, is the measure to watch.

There is a smarter approach. Since its inception, the Trefis High Quality (HQ) Portfolio has returned 105%, beating the benchmark that combines the three major indices - the S&P 500, S&P Mid-cap, and Russell 2000. And it did so without taking the concentrated risk that comes with do-it-yourself stock picking. If that is how you want to invest, the HQ Portfolio is the place to start.
