Dawne Eldridge Named Colleague of the Month for MH Ambulatory Group

Dawne Eldridge, a transitional care manager with Memorial Care Coordination Services, has been named October Colleague of the Month for the Memorial Health Ambulatory Group.

Eldridge works closely with patients identified as high risk for hospital readmission. She follows patients with complex health needs after they leave the hospital, providing an intensive, easy-to-follow care plan for 45 days to support their recovery and help them avoid unnecessary readmissions.

Her support extends to her colleagues as well. Eldridge is known as a dependable resource, answering questions, helping provide coverage and responding to patient needs. She keeps patient safety and quality care at the center of her work.

"Whether it is answering a question, assisting with coverage, helping her colleagues or patients, she is always right in the thick of it," wrote the nominator. "She is truly valued."
