Investors holding Centene (CNC) stock over the year to November 4, 2026, more than doubled their money. The shares returned 110.5%, against 14.9% for the S&P 500. That includes a 12.6% jump on November 4, 2026, when Medicaid insurers rallied on the midterm election results. Over that year, the Medicaid insurer returned to a profit. Management describes this recovery as restoring its margins, but in the six months before that year the stock had lost 42.3%. Was that recovery visible while the shares were still falling?
Centene Flagged Rising Medical Costs In July 2025
Partly. On its July 25, 2025 call, management called a 94.9% health benefits ratio, representing the share of premiums paid out in medical costs, "unanticipated and unacceptable". The company named behavioral health as the biggest reason that ratio rose from the quarter before, and it pointed to a problem in its marketplace business that it had flagged earlier that month.
For the quarter ended June 2025, Centene posted an operating loss of $403 million, following $1.53 billion of operating income in the quarter ended March 2025. On its own, that was bad news. But the two problems management named are the ones Centene later reported getting better. On its October 27, 2026 call, the company reported another quarter of steadier behavioral health costs and said the marketplace business had outperformed in the quarter.
Centene's Operating Loss Shrank By September 2025
The next filing on October 29, 2025, showed a $211 million operating loss for the quarter ended September 2025. That was about half the loss of the quarter before, but it was still a loss. The same quarter of 2024 had brought $665 million of operating income.
That same day, Centene reported a loss of $13.50 per share. The company said that loss came from a $6.7 billion non-cash goodwill write-down. At $6.63 billion, the quarter's net loss was far larger than its $211 million operating loss. The turn had not yet arrived: the quarter ended December 2025 brought a wider operating loss of $1.23 billion. By the March 2026 quarter, operating income was back at $1.86 billion.
How Much Profit Centene Had Restored By 2026
Centene earned $1.20 billion of operating income in the quarter ended June 2026, compared with a $403 million operating loss a year earlier. Operating income reached $1.52 billion over the four quarters to September 2026, up from $1.10 billion a year earlier. That still left an operating margin of 0.7%, below the 1.8% of the four quarters ended September 2024.
Elevance Health, the best of the three peers measured over the same dates, returned 39.9%, or about a third of Centene's gain. UnitedHealth Group returned 27.0%, and CVS Health returned 5.1%.
Is Centene Still Repairing Its Margins?
Management says it is, but the latest reported quarter was weaker. On the October 27, 2026 call, management raised its 2026 adjusted earnings guidance to more than $5.20 per share from more than $4.80. It said the Medicaid health benefits ratio improved to 94% in the third quarter, coming in 30 basis points better than in the second.
Centene posted an operating loss of $307 million in the quarter ended September 2026, wider than the $211 million loss a year earlier. Management said the marketplace business's underlying results that quarter essentially helped pay for a bigger reserve for billing disputes with providers.
Medicaid rates reset next. Just over 40% of Centene's Medicaid revenue gets new rates on January 1, and management said it sees signs those rates will better match its costs. The company's stated worry for 2027 is a gap between rates and how sick its members are, as work requirements take some members off the rolls.
Centene is expected to report in February 2027, when it plans to give its 2027 guidance. Its Medicaid health benefits ratio is the number to watch in that report. Management expects to grow earnings in 2027, so on its account the story has not run out. The February report will show whether the results agree.
