The Centers for Medicare & Medicaid Services (CMS) is answering Frequently Asked Questions (FAQs) about the Hospice Quality Reporting Program (HQRP), including the Hospice Outcomes and Patient Evaluation (HOPE) tool, HQRP quality measures, data submission requirements, and reports.
