Nuclear safety deficiencies that occurred between 2019 and 2022 revealed multiple violations of DOE nuclear safety requirements by Mid-America Conversion Services, LLC at Portsmouth Paducah Project Office.
On January 15, 2026, the U.S. Department of Energy (DOE) Office of Enterprise Assessments' Office of Enforcement issued a Final Notice of Violation (NEA-2026-01) to Mid- America Conversion Services, LLC (MCS) for violations of nuclear safety requirements (10 C.F.R. 820 and 830) related to inadequate hazards analyses, inadequate causal analyses of events, inadequate processes for planning and scheduling work, and the performance of work by unqualified workers at the Portsmouth Paducah Project Office.
