Ashley, a HouseCalls nurse practitioner in Louisiana, saw this firsthand. During a HouseCalls visit, she found her patient living in a partially burned home with no electricity or running water. These unsafe conditions were directly impacting the patient's health, yet they may not have been identified during a typical doctor's office visit.
Thanks to HouseCalls, Ashley connected the patient with local resources to secure safe housing - one of over 553,000 referrals made through the program to address social drivers of health.
