NTSB Issues Investigative Update on B-767 Runway Excursion Accident in…

​​WASHINGTON (Sept. 9, 2026) - The National Transportation Safety Board is providing the following update on its investigation into the Sept. 6 runway excursion accident involving 21 Air Flight 7598, a Boeing 767-33A cargo airplane, at Miami International Airport.

The following information from the cockpit voice recorder (CVR) and flight data recorder (FDR) is preliminary and subject to change.

  • The NTSB recovered the CVR and FDR from the accident site and transported them to the NTSB's Vehicle Recorder Laboratory in Washington, DC, where they were successfully read out.
  • The aircraft was equipped with an Acron Aviation (formerly L3Harris) model FA2100 CVR and an Ontic (formerly Honeywell) model SSFDR FDR.
  • The CVR contained more than 2 hours of good quality audio across four channels.
  • The FDR contained approximately 54 hours of data and recorded more than 400 parameters.
  • The NTSB will convene a CVR group in the near future at NTSB headquarters in Washington, DC, to review the accident flight recording and produce a written transcript.

A summary of select events captured on the CVR during the final few minutes of the recording is provided below. All times are referenced relative to the end of the CVR recording. The times and descriptions are preliminary and subject to change.

Note that the CVR and FDR times have not been precisely synchronized. The times and descriptions are preliminary and subject to change.

The nose and right main gear touched down at a groundspeed of 158 knots.

​The brakes were applied at a groundspeed of 146 knots.

​The left main gear touched down at a groundspeed of 134
knots.​

​Brakes were released at a groundspeed of 120 knots and throttles were increased to a value consistent with go-around thrust.

​The throttles were reduced to idle power and the brakes were re-applied. The groundspeed was 117 knots. The throttles remained at idle, and brakes remained applied until the end of the recording.

​Recorded lateral, longitudinal, and vertical acceleration values
changed markedly; the FDR data indicated a groundspeed of 96 knots.

​The FDR recording ended. The final recorded groundspeed was 65 knots. There was no indication in the recorded data that speed brakes or thrust reversers were deployed.​
