Advance Care Planning Workshop - Birkenhead Library

This workshop explores advance care planning - a process of thinking and talking about your values, goals, and preferences for current and future health care, including end-of-life care.

Participants will learn how to start advance care planning conversations, discover available tools and resources, identify key people to involve, understand how to record and share their wishes, and gain confidence in preparing for future healthcare decisions.
