Guardian Pharmacy Supports the PACE Model of Care at the National PACE Association Conference

Guardian Pharmacy joined providers, payers, and policymakers at the National PACE Association (NPA) Conference in New York City, connecting with leaders advancing the Program of All-Inclusive Care for the Elderly (PACE). The event, held October 5 - 8, highlighted collaborative approachesthat help older adults live safely in their communities through integrated, team-based care.

Guardian's Role in Supporting NPA

As a long-term care pharmacy partner, Guardian contributes to medication safety, access, and education that strengthen PACE outcomes-helping reduce preventable hospitalizations, improve adherence, and support individualized care plans.

Representing Guardian at the conference were Rockford Anderson, PharmD, President, Right Dose Pharmacy, and Darla Spears, PharmD, RPh, BCGP, Clinical Consultant Pharmacist with Boomer Solutions. Both attended sessions and met with PACE leaders from across the country to exchange best practices in medication safety, participant-centered logistics, and community-based pharmacy collaboration.

"NPA provides excellent educational opportunities including evidence-based best practices for improving outcomes for participants in areas that really matter, like fall prevention and reducing admissions to hospitals and psychiatric care facilities," Spears said.

Anderson emphasized the value of local engagement.

"Our local pharmacy teams work closely with the communities we serve, giving PACE programs a partner who's collaborative, responsive, and invested in their participants," he said.

Conference attendees also toured PACE centers in the New York City area, observing participant-focused care models in practice.

"This conference offered valuable opportunities to connect with peers who share a commitment to helping participants thrive in their communities with dignity and grace," Spears added.

Continued Support for the PACE Model

Earlier this year, Guardian participatedin the PACE Spring Policy Forum in Washington, D.C., joining discussions with members of Congress, federal agencies, and industry leaders on policies shaping the future of the PACE model of care.
