What Will Decide UnitedHealth Group Stock Over The Next Six Months

For questions about this article or underlying research, email [email protected].

UnitedHealth Group (UNH) stock returned 21.3% over the six months to October 22, 2026, compared with 8.4% for the S&P 500. On October 13, management raised its 2026 earnings guidance and called the low end of its 13%-16% long-term growth range a reasonable start for 2027. For that outlook to hold, the company's Medicare plans must keep recovering and the costs in its commercial health plans must stop running ahead of plan. Below are the events most likely to move UnitedHealth Group stock over the next six months.

What to watch When Which way it cuts
Medicare annual enrollment for 2027 plans October 2026, opened the week of the October 13, 2026 call Either way
Fuller outlook for 2027 from management before the end of 2026 Either way
Debt-to-capital ratio, 41.7% at the end of Q3, brought toward 40% by the end of 2026 In its favor
Fiscal Q4 2026 results January 2027 Either way
Commercial plan costs above plan, priced into 2027 fiscal year 2027 Against the stock

Medicare Enrollment For 2027

Seniors are choosing their 2027 Medicare plans now. The annual enrollment period opened the week of UnitedHealth Group's October 13 call. Medicare is the part of the recovery that is holding, and management expects Medicare margins near the top of its 2%-4% range for 2026.

That recovery has cost members. The company's pricing and benefit changes have shrunk its Medicare Advantage membership, and in July management expected a decline of about 1.1 million for 2026. These plans sit inside UnitedHealthcare, the insurance business that booked $345 billion of revenue in 2025. Management says it priced 2027 plans with the same discipline and will favor HMO and special needs plans. Medicare Advantage membership is the measure to watch when UnitedHealth Group reports in January 2027.

Management's Outlook For 2027

Management plans to issue a fuller outlook for 2027 before the end of 2026. Commercial plan costs are the part of the recovery that is not holding yet. Management said cost trends in those plans are running above the 11% it first expected, driven by disputes under the No Surprises Act, more intensive care from clinicians and costly specialty drugs.

The company priced 2027 plans on the view that those trends will persist, and said this will stretch out the time it takes to reach its commercial margin targets. Management will base that outlook on 2026, a year for which it guides adjusted earnings of at least $20.50 a share and GAAP net earnings of at least $19.50 per share. The fuller 2027 outlook, due before year-end, is the update to watch.

UnitedHealth Group's January 2027 Earnings Report

UnitedHealth Group is expected to report its fiscal Q4 2026 results in January 2027, and its 2026 earnings will include spending management has already planned. The 2026 guidance of at least $20.50 in adjusted earnings per share already accounts for investment in the final months of the year. Management also cited faster investment in UnitedHealthcare and Optum Insight as a headwind for 2027.

The medical care ratio, which tracks the share of premiums paid out for care, shows how costs are running. The ratio stood at 88.4% in fiscal Q3 2026, compared with 89.9% in fiscal Q3 2025. The last January report brought a sharp move: the stock fell 16.4% around the company's January 27, 2026 call.

Can You Hold UnitedHealth Group Stock Through These Dates?

Owning these shares has been difficult before. Over the 103 trading sessions that ended on March 27, 2026, the stock fell 29.6% from its October 28, 2025 close.

The Medicare recovery is holding today, while the commercial cost problem is not fixed. Commercial plan costs are the weaker of the two, and the medical care ratio in the January 2027 report is the measure to watch for them. If a fall like that 29.6% drop would force you to sell, a smaller position fits these six months.
