04/01/2026 - REMINDER: Effective April 1, 2026, faxed submissions of claim-related forms — including IME reports — are no longer accepted

Effective today, April 1, 2026, the New York State Workers' Compensation Board (Board) is no longer accepting claim-related paper forms and documentation via fax. The Board's fax number for claims is discontinued, and faxed submissions are no longer an accepted method of submission for claim-related documents. This includes Independent Medical Examination (IME) reports.

Claim-related documents may be submitted to the Board by regular US Postal Service mail, email, electronic upload, web filing, and xml form submission. Visit the How to Submit Claims-Related Forms and Documents webpage for more details.

Note: Faxed documents will still be accepted by the Board's Employer Compliance and Alternate Dispute Resolution units.
