Join the ACL-funded National Alzheimer's and Dementia Resource Center webinar, "Bridging Health Care and Social Services for People Living with Dementia and their Caregivers."

As the number of people living with dementia (PLWD) continues to increase, addressing their needs - and those of their caregivers - has become more important. Coordination of comprehensive care and support services across community-based organizations and health care systems is necessary to achieve better health outcomes, which allow for both PLWD and their caregivers to remain in their homes and communities.

In this webinar, panelists will discuss how their organizations integrate health care and social services to better support PLWD and their caregivers. They will highlight successful program elements, share challenges faced in implementation and expansion, and offer recommendations for health care systems and community-based organizations working to bridge gaps between medical health care and social services.
