Falls Community Health is joining community health centers nationwide in recognizing National Health Center Week, Aug. 2-8, by highlighting the role it plays in providing affordable, comprehensive health care for individuals and families in Sioux Falls.
Falls Community Health offers primary medical care, dental care, behavioral health services and care coordination for patients of all ages. The clinic serves insured, uninsured and underinsured patients and accepts most private insurance plans, Medicare and Medicaid. Patients who qualify may receive care through a sliding-fee discount program based on household income.
"National Health Center Week provides an opportunity to raise awareness about the services available through Falls Community Health and the importance of accessible health care in our community," said Joe Kippley, public health director for the City of Sioux Falls. "Our team is committed to providing quality, comprehensive care while helping patients navigate their health care needs."
Falls Community Health, located at 521 N. Main Ave. in Sioux Falls, offers the following services:
In addition to its downtown Sioux Falls clinic, Falls Community Health operates school-based health clinics and a mobile dental bus. Each clinic is staffed by an advanced practice provider, a nurse and dental staff members and provides care to students with parent or guardian consent.
Additionally, Falls Community Health provides outreach, education and support for individuals experiencing homelessness. Patients can access medical care, case management and referrals to community resources for health care and other essential services.
To help make health care more affordable, Falls Community Health offers a sliding-fee discount program based on federal poverty guidelines. Eligible patients may qualify for reduced costs for medical and dental services after providing proof of household income.
In addition to employing five providers directly, Falls Community Health partners with the Center for Family Medicine to provide patients access to experienced faculty physicians and family medicine resident physicians who train under physician supervision.
