Centene (CNC) stock returned 104.2% in the year to October 23, 2026, while the S&P 500 returned 15.1%. The shares recorded their largest one-day gain of that year, jumping 14.0% on April 28, 2026, when the health insurer reported first-quarter results and raised its 2026 earnings outlook. How big is the profit recovery behind that outlook, and which parts of Centene's business is it coming from?

Centene Now Keeps More Of Every Premium Dollar

Centene now spends a smaller share of its premiums on members' medical costs. Across the company, that share fell to 89.6% in the second quarter of 2026, down from 93% a year earlier, management said on the July 28, 2026 call. As a round example, an insurer that pays out 90 cents of every premium dollar keeps 10 cents for running costs and profit, meaning a few points make a large difference. Alongside that lower share, Centene reported net income of $1.09 billion in the second quarter of 2026, against a loss of $0.253 billion a year earlier.

Centene's peers did not keep pace in the stock market. UnitedHealth Group, Elevance Health and CVS Health returned between 18.8% and 29.6% over the same year.

Where Has Centene Improved The Most?

The sharpest improvement came in the Commercial segment, which management said is led by Marketplace plans. The segment's share of premiums spent on medical costs fell to 79.2% in the second quarter of 2026, from 90.6% a year earlier. Part of that drop was one-time: a final settlement of 2025 risk-adjustment payments added a $481 million pre-tax benefit in the quarter. Commercial is not Centene's largest business, generating $42.0 billion in revenue out of $195 billion for the whole company in 2025.

Management now expects the Marketplace business to earn a pretax margin of 4.5% to 5% in 2026 on its own adjusted measure, better than both of its earlier guides. Citing strong first-half results, management also raised its full-year 2026 adjusted earnings per share outlook on the July call to more than $4.80, up from more than $3.40 in April.

Medicaid, Centene's Largest Business, Is Improving More Slowly

Medicaid brought in $110 billion of that $195 billion in 2025. The segment's share of premiums spent on medical costs was 93.9% in the second quarter of 2026, down from 94.9% a year earlier, a far smaller gain than in Commercial. Management still expects that figure to land around 93.5% for the full year, the same forecast it gave before.

Medicaid is also losing members. Management now expects full-year Medicaid membership to fall 8% to 9% from the end of 2025, against its earlier view of a 6% fall. So the recovery behind the move is led by the smaller Commercial business, while Medicaid, the larger one, is improving more slowly and losing members. Centene is expected to report its third-quarter results on October 27, 2026, and its Medicaid membership is the number to watch in that report.

How To Act On CNC?

Not necessarily on Centene alone, if your goal is long-term wealth. Wealth is built by owning the companies that keep outperforming, the leaders of their industries, wherever they are. Here is one company from Centene's own industry that has beaten it. Over the five years to October 2026, HealthEquity stock rose 54%, while Centene stock rose 1.3%.

Finding companies like these takes comparison across industries and sectors, and that is what the Trefis High Quality (HQ) Portfolio does. It holds 30 quality companies in equal weight and, since its inception in September 2020, has returned more than 105%, ahead of a benchmark that combines the S&P 500, S&P Mid-cap and Russell 2000. See how we find the leaders.
