ABA Authorization Fax and Email Submissions Ends December 7, 2026

Effective December 7, 2026, Horizon will no longer accept Applied Behavior Analysis (ABA) authorization requests by fax or email. Any ABA authorization request submitted by fax or email after this date will not be processed. Instead, providers should submit ABA authorization requests electronically through Availity Essentials™.

Submit ABA authorization requests through Availity now to prepare for this change.

You must have an Availity account to use the authorization tool.

If your organization is already registered with Availity Essentials but you do not yet have an account, contact your organization's Availity Administrator to request access.

If your organization is not yet registered or you need additional guidance, visit Register and Get Started with Availity Essentials for registration information and resources.

If you're unable to access Availity Essentials, please call Availity Client Services at 1-800 AVAILITY (282-4548).

To Access the Authorization Request Tool

1. Log in to Availity Essentials.
2. Under Payer Spaces, select the appropriate network.
3. Select Applications.
4. Select Utilization Management Requests.
