Queen’s Announces Statewide Expansion of Hoʻopili Ola Remote Patient Monitoring Program

The Queen's Health Systems (Queen's) announced the statewide expansion of its Hoʻopili Ola Remote Patient Monitoring (RPM) program, bringing home-based care and support to eligible patients across Hawaiʻi living with chronic conditions such as diabetes, hypertension, congestive heart failure, COPD, and obesity.

The statewide expansion is supported by a new federal grant through Hawaiʻi's Rural Health Transformation Program under the Pili Ola initiative, supported by the Office of Governor Josh Green. The expansion builds on the success of Queen's pilot RPM program, which launched in March 2025 to serve Native Hawaiian patients in rural communities. The statewide rollout now broadens access, allowing eligible patients of all ethnicities across the islands to receive care from the comfort of home.

"We are grateful for the federal funding through Hawaiʻi's Rural Health Transformation Program, which allows us to carry forward the vision of our founders, Queen Emma and King Kamehameha IV, to improve the health and well-being of Native Hawaiians and all the people of Hawaiʻi," said Darlena Chadwick, Executive Vice President and Chief Operating Officer of The Queen's Health Systems. "Hoʻopili Ola allows us to extend high quality, compassionate care beyond the walls of our hospitals and into homes and communities across the state."

How Hoʻopili Ola Works

Through the Hoʻopili Ola RPM program, patients receive a tablet and Bluetooth-enabled devices to track health data like blood pressure and weight, which send information directly to care teams in real time. The technology uses its own cellular connection, so home internet is not required.

The name Hoʻopili Ola reflects connection, health, and well-being. It signifies bringing together patients, healthcare providers, and technology to support better health outcomes and a more connected care experience.

Nurses monitor patients 24/7 and respond quickly to critical alerts, often identifying issues before they become emergencies. Patients also receive support from Queen's Patient Community Navigators, who provide education, home visits, and connections to additional services such as pharmacy, dietitian, social work, and behavioral health resources.

A Life-Changing Impact: Kamaunu's Story

For Nānākuli resident Camellia "Ulu" Kamaunu, the program has been life-changing.

"I was struggling," Kamaunu said. "My highest weight was 342. I am now 187, and it feels fantastic!"

Kamaunu said the daily accountability and regular check-ins help keep her on track.

"If I didn't take my blood pressure or weigh myself, somebody called me," she said. "That is the support that keeps you in line."
