Ascension Wisconsin and Outreach Community Health Centers Launch Mobile Health Clinic to Serve Medically Underserved in Milwaukee

Ascension Wisconsin and Outreach Community Health Centers have launched an essential community health initiative through the Outreach Community Health Centers Mobile Health Clinic, in collaboration with Ascension Wisconsin. This collaborative effort is designed to meet a critical community need by delivering high-quality, preventive and acute care directly to vulnerable and medically underserved residents in Milwaukee.

Both Ascension St. Ben's Clinic and Outreach Community Health Centers share a long history of serving those most in need in the area, addressing the significant barriers patients often face, such as lack of insurance, money and transportation.

The mobile unit will be located in neighborhoods with the greatest need, establishing trusted relationships by meeting patients where they are. Each visit will begin with compassionate listening, delivering essential healthcare and offering hope.

Comprehensive Healthcare Services

"Far too many of our neighbors struggle with transportation, lack of insurance, financial hardship, or other obstacles that prevent them from receiving the care they need. This mobile clinic removes many of those barriers by bringing high-quality primary care directly into neighborhoods where access has been limited," said Julia Harris-Robinson, CEO/President, Outreach Community Health Centers.

An Ascension Wisconsin family medicine nurse practitioner and nurses will provide acute and preventative care for adults. An Outreach Community Health Centers patient services representative will assist with scheduling and care coordination. Services provided will include:

• Medical care and treatment for common illnesses like cough, sore throat, rash, etc.
• Management of chronic diseases like high blood pressure and diabetes
• Medication management
• Vaccinations
• Care navigation and referrals for behavioral health, prenatal care, specialty care, mammography and dental services
• Access to community resources and social services

Addressing Community Disparity

This initiative targets areas identified as high disparity zones with no immediate primary care access, demonstrating Ascension Wisconsin's and Outreach Community Health Centers' commitment to providing access to quality healthcare for all.

"For more than four decades, Ascension St. Ben's Clinic has provided primary and preventive healthcare services to people who are struggling with homelessness or at risk of homelessness," said Nichole Gladney, Community Health Services and Engagement Director, Ascension Wisconsin. "The addition of this mobile unit will expand those critically important services into more Milwaukee neighborhoods and help our community flourish."

Mobile Clinic Schedule

To start, the mobile clinic will be available at the following locations:

• Tuesdays 10 a.m. - 4 p.m. at Calvary Baptist Church
• • 2959 N. Teutonia Avenue, Milwaukee, WI 53206
• Thursdays 10 a.m. - 4 p.m. at Ascension Ebenezer Health Resource Center
• • 3132 N. Dr. Martin Luther King Jr. Drive, Milwaukee, WI 53212
  • • The Ascension Ebenezer Health Resource Center Food Pantry will be open during this time
