2025 Community Health Needs Assessment reveals region's top health needs

The assessment is released every three years.

The 2025 Community Health Needs Assessment (CHNA), a report identifying the top health issues and needs of our region, has been published. The assessment is released every three years and is used to guide Community Health System's efforts to improve the health and well-being of Central Valley residents.

The key health needs identified in the 2025 report are:

  • Access to Healthcare, including maternal healthcare
  • Chronic Diseases, including weight status
  • Economic Stability, focusing on food insecurity and homelessness
  • Mental Health & Disorders, including substance use disorder

Who is involved: The report is a collaborative effort between Hospital Council of Northern and Central California and seven regional hospitals and public health department partners.

Nearly 5,000 community members, leaders and key stakeholders representing low-income and vulnerable populations provided primary data through an online survey, focus groups, and interviews. Secondary data reported by federal and state sources was also referenced.

The CHNA covers Community's service area: Madera, Fresno, Kings and Tulare counties.

Why it matters: Assessing the impact of interventions made in previous years is necessary in strategic planning and decision making for future programs and resources. As outlined in The Cause, our aim is to prioritize preventive health measures to make the community safer and healthier.

The CHNA gives us a guideline to focus on where those efforts are most needed.
