Horizon Specialty Care (HMO C-SNP) Plan Coming in 2027

We are pleased to share that beginning January 1, 2027, Horizon Healthcare of New Jersey, Inc. (Horizon) will offer Horizon Specialty Care (HMO C-SNP), a Medicare Advantage chronic condition special needs plan designed to support members with certain chronic conditions. These conditions will initially include diabetes, congestive heart failure and cardiovascular disease.

In 2027, the plan will be available in Bergen, Essex, Hudson, Morris, Passaic, Somerset and Union counties. Providers who participate in both our current Medicare Advantage (Braven Health) plans and our current Horizon NJ TotalCare (HMO D-SNP) plans will be participating providers and included in the Horizon Specialty Care (HMO C-SNP) network.¹

You will be able to confirm your participation status with Horizon Specialty Care (HMO C-SNP) plans on or after October 1, 2026 through the Doctor & Hospital Finder.

Horizon Specialty Care (HMO C-SNP) participating providers will reimbursement at Medicare Advantage (Braven Health) contracted rates. Horizon Specialty Care (HMO C-SNP) fee schedule information will be available through Availity Essentials^TM beginning January 1, 2027.
