Keeping your information current helps ensure that Horizon can accurately reflect your ASC relationships, making it easier for members and referring providers to identify care options connected to your practice.

How to Review and Validate Your ASC Affiliations

• Log into Availity.com
• Select Horizon Blue Cross Blue Shield from the Payer Spaces dropdown.
• Select the Application tab.
• Click on the ASC Affiliation tile.
• Review your ASC affiliation information and confirm that it is accurate and up to date. If you do not currently have any ASC affiliations, simply confirm that status in Availity.

While ASC affiliations are not required, establishing appropriate ASC affiliations can enhance members' access to care, and promote care in the most appropriate setting.
