CMS Releases 2025 MIPS Final Scores and Opens Targeted Review Board

CMS recently announced the release of Merit-based Incentive Payment System (MIPS) performance feedback and final scores for the 2025 performance year. Clinicians who believe their final score was calculated incorrectly may now request a targeted review.

The targeted review period will close 30 days after CMS releases the 2027 MIPS payment adjustments associated with 2025 performance. These are expected later in October. APMA encourages members to review their scores and gather supporting documentation if they identify potential errors. Targeted review decisions are final and are not eligible for further review.

To access performance feedback or submit a targeted review, MIPS eligible clinicians should sign in to the Quality Payment Program (QPP) website using their Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) credentials. Additional guidance is available in the QPP Resource Library.

Additional MIPS Deadlines: Clinicians planning to report through a MIPS Value Pathway (MVP) for the 2026 performance year must register through the QPP portal by November 30. Registration must be completed by the clinician or practice, not an EHR vendor or registry. CMS is expected to release the final rule establishing 2027 MIPS requirements this fall.
