As previously announced, HealthCheck360 performs ALL prior authorization (PA) reviews for members enrolled in UNITE HERE HEALTH (UHH) plans covered by Horizon.
- Please call HealthCheck360 at 1-844-462-7812 to initiate a PA request for any of the procedures/services listed on our UNITE HERE HEALTH webpage.
- Please do not submit any PA requests for UHH members through our online Utilization Management Request Tool.
