Community Subacute & Transitional Care Center has received a 5-Star Quality Measures Rating - and an increased overall rating of 4 stars - from the Centers for Medicare and Medicaid Services (CMS).

The 5-Star Quality Rating System is a national benchmark that reflects the highest standards in patient care. CMS assigns one overall rating plus separate ratings for health inspections, staffing, and quality measures - a term used to describe the effectiveness of a hospital to follow protocol, prevent harm, and provide exceptional patient outcomes.

Caring for patients who need high-quality, ongoing care

Located in Central Fresno, Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for chronic, subacute conditions that require 24-hour medical supervision, e.g. coma or other complex illnesses.

Achieving a CMS Five-Star Rating isn't just about recognition for the facility - it's about Community's commitment to forwarding The Cause every day. The rating reflects our team members' commitment to exceptional care, compassion and trust, and our mission to make the Central Valley healthier for all.

"The recognition of Community Subacute Transitional Care Center is a testament to the expertise, vigilance and compassion our team members bring to every patient, every time," said Danny Davis, Division President, Hospitals, Community Health System. "It's also a reminder for us to keep striving to provide the highest quality of care for patients in the Central Valley - because they deserve it."
